Elder Care Option Provides
A One-Shop for Aging Family Members
Elder Care Options, a new program of Family and Children’s
Services of Central Maryland, helps caregivers find all
the services and resources they need to care for elderly
family members. Designed to provide high quality services
that assist with long-term health and security of older
adults while allowing them to continue living with dignity,
Elder Care Options provides Care Managers who coordinate
everything from legal services to medical care.
Elder Care Options care managers are licensed social
workers or registered nurses with in-dept knowledge of
Central Maryland, its residents and many resources. Services
are tailored to the client’s needs.
Care Managers help families and older people by navigating,
coordinating and supporting the client and family through
medical crises and discharge planning; conducting assessments
focusing on the total person; developing a written summary
of the client’s status with Care Plan recommendations,
arranging and monitoring services to implement the Care
Plan recommendations; coordinating with attorneys, financial
planners, insurance agents and medical professionals;
being available when family caregivers need a break,
go on vacation or live far away from their loved one;
and providing education, advocacy, counseling and support.
Care Managers provide monthly monitoring and care plan
updates and are available for crisis intervention and
reassessments.
Family and Children’s Services’ Elder Care
Options program is accredited by the Council on Accreditation
of Services for Families and Children, Inc., licensed
by the Maryland Department of Mental Hygiene, Office
of Health Care Quality, and is a member of the National
Association for Home Care and the United Way of Central
Maryland.
